Correction of enlarged male breasts What is it? A surgery that removes fat and or glandular tissue from the breasts, and in extreme cases removes excess skin, resulting in a chest that is flatter, firmer and better contoured. What does it accomplish? The chest is flatter, firmer and better contoured. What happens during the first consultation? Dr. Adams will discuss your surgery and determine if you are a good candidate. He will examine your breasts and check for causes of the gynecomastia, such as impaired liver function, use of estrogen-containing medications, or anabolic steroids. If a medical problem is the suspected cause, you'll be referred to an appropriate specialist. Your plastic surgeon may, in extreme cases, also recommend a mammogram, or breast x-ray. This will not only rule out the very small possibility of breast cancer, but will reveal the breast's composition. Once your surgeon knows how much fat and glandular tissue is contained within the breasts, your surgeon can choose a surgical approach to best suit your needs. You will have the opportunity to view pre and post-op pictures, be photographed and discuss pre and post-op care. Dr. Adams assistant will give you the total fee, which includes the fees of the surgeon, anesthesia, and operating room. and will give written pre and post-op care and informed consent to review before signing and discuss your consultation appointment. How is it done? If your gynecomastia consists primarily of excessive fatty tissue, your surgeon will likely use liposuction to remove the excess fat. A small incision, less that a half-inch in length, is made around the edge of the areola--the dark skin that surrounds the nipple. Or, the incision may be placed in the underarm area. A slim hollow tube called a cannula, which is attached to a vacuum pump, is then inserted into the incision. Using strong, deliberate strokes, the surgeon moves the cannula through the layers beneath the skin breaking up the fat and suctioning it out. The advent of ultrasonic assisted lipoplasty has dramatically improved the results of suction to decrease the gynecomastia and cause skin retraction to assist the skin to shrink back up to re-adjust your new breast contour. In extreme cases where large amounts of fat or glandular tissue have been removed, skin may not adjust well to the new smaller breast contour. In these cases, excess skin may have to be removed to allow the remaining skin to adjust to the new contour. What can I expect after surgery?
Recovery time: You will feel tired and sore for 1-3 days following your surgery. Your surgery may be done on an outpatient basis, there may be exceptions requiring an over night stay in the hospital. Your surgeon will decide based upon the extent of surgery you require. You will be restricted in physical exercise for the first four weeks. You will be wearing a compression garment for two weeks, 24 hours a day and the next two weeks during the night or day. You will wake up wearing a compression garment for 2 weeks day/night then the next 2 weeks during the night. You may wear day/night if you are able. You will be seen for follow up in 3-4 days. You may return to work in 7-10 days with limitations on your physical activity. No lifting over 10 pounds for 4 weeks. No aerobic workouts to increase your heart rate above 100 beats per minute for 4 weeks. Walking may begin the day of surgery. How many visits do I have after surgery? You will follow-up with the physician during the first week and then every two to three weeks for the first six weeks. After this time you will be seen approximately every 4 months for the first year. Am I a good candidate? Your visit with your surgeon will determine if you are a good candidate. If you are in good physical and mental health, have no breast disease with reasonable expectations then you would be an appropriate candidate. What does the surgery cost? You will be given a quote that includes fees for the surgeon, hospital, anesthesia and lab. How do I pay for surgery? The surgery costs are discussed with you following your visit with your plastic surgeon. You will be given an estimate for the surgeon's , hospital and anesthesia fees. For your convince the fees will be paid in one sum prior to your surgery, at least 2 weeks in advance. Checks, money orders, cash and credit cards can be used for payment. Are there scars ? Yes. Whenever surgery is performed on your body a scar will result. The incisions are placed in the areas to make the scar the least noticeable. Your surgeon will discuss the placement of incisions during your first visit. Does it hurt? (1) You will have plenty of pain medications for discomfort after surgery. Please tell us of all your drug allergies. Some pain medication may cause nausea this should not be an allergy. All pain medication will cause drowsiness and therefore you should not drive yourself and or operate equipment. (2) An antibiotic is used for only a few days after your surgery. (3) Sometimes following surgery to help decrease swelling your surgeon may prescribe Medrol -dose pack. Will I be awake during surgery? Your surgery is best done under general anesthesia for control of pain and nausea if they develop post surgery. You will be entirely asleep for the surgery. Where is the surgery performed? Your surgery is performed in Zale Lipshy Hospital outpatient surgery center with no over night stay. There may be an exception that requires an over night stay in the hospital and Dr. Adams will decide based on the extent of your surgery required. Are medical photographs taken? Photographs will be taken during your first visit for your surgeon and then after your surgery at about two months. Medical photographs are essential to help your surgeon better assess your body's contouring requirements to assist in planning your surgery. What medications should I avoid? You should avoid or be off all diet or over the counter weight medications. The only homeopathic medications are those recommended by Dr. Adams. It is especially important to be off St. John's Wort and Ginseng, which can cause adverse effects during anesthesia. Please stop taking all aspirin-like products, Advil, and ibuprofen as these medications can cause bleeding problems. Please tell Dr. Adams what medications you are on ( prescription or non-prescription) to ensure the safety of any surgery.
